ONCE YOU HAVE CLICKED ON THE ABOVE LINK PLEASE FOLLOW THE BELOW INSTRUCTIONS:

Step 1: On the left hand side navigation, click on the Addittions

Step 2: Decide if you want to do a quick entry or a more detailed entry. Once you have made that decision select either
Add Marker: Simple
Add Marker:Detailed

Step 3: This will then open up a window where you will enter in your personal information. Once complete, click on Preview and then Submit! It really is that easy!
